Procedures

Reducing stomach capacity

The sleeve gastrectomy involves removing a significant portion of the stomach to create a narrower, tube-shaped stomach. The intestinal tract remains unchanged.
In addition to reducing the amount of food that can be consumed, the procedure can influence certain hormonal mechanisms involved in hunger and satiety.
The sleeve is one of the procedures that can be evaluated during an initial bariatric surgery project. The presence of reflux or other medical factors can notably influence the surgeon’s evaluation.

An approach without abdominal incisions

In certain situations, an endoscopic sleeve can be considered to reduce stomach volume using endoscopic sutures, without removing part of the stomach and without abdominal incisions.
In someone who has already had a surgical sleeve, an endoscopic approach can sometimes be evaluated to reduce the volume of a stomach that has expanded over time. This is sometimes referred to as a resleeve or endoscopic revision.
However, this approach is not suitable for all situations. The surgeon must evaluate the anatomy, previous surgery, reasons for weight regain, or present symptoms to determine whether an endoscopic procedure can be considered.

Modifying the stomach and digestive tract

Gastric bypass involves creating a small stomach pouch that is then connected to a more distant section of the intestine. Food thereby follows a modified digestive path.
This procedure notably combines a reduction in stomach capacity with digestive and metabolic modifications.
It can be considered as a primary surgery or, in certain situations, as part of a conversion following a previous bariatric surgery.

Another digestive bypass approach

The mini-bypass, also called one-anastomosis gastric bypass, involves creating a narrower stomach that is connected to the intestine in order to alter the passage of food.
While it shares certain principles with the gastric bypass, its surgical configuration is different.
As with other procedures, its benefits, limitations, and key considerations must be evaluated according to each individual’s situation.

An approach that modifies the path food takes

Intestinal bipartition combines a sleeve gastrectomy with a modification of the intestinal pathway. A new path allows a portion of the food to reach a more distant section of the intestine, while also maintaining passage through the natural digestive tract.
It can be considered in certain situations based on the individual’s goals and medical profile.
This approach aims to combine different mechanisms that can contribute to weight loss and the metabolic effects of the surgery.

A procedure combining a sleeve with a major intestinal bypass

SADI-S combines a sleeve gastrectomy with a major alteration of the intestinal pathway. It is a procedure whose effects on absorption and digestive function differ from those of a sleeve alone. Therefore, it involves specific considerations, particularly regarding diet, supplementation, and long-term follow-up. The surgeon must evaluate whether this approach may be appropriate based on the individual’s medical profile and goals.

Revision or conversion

Having already had bariatric surgery does not necessarily mean that all options have been exhausted.
Over the years, various situations may lead an individual to request a new evaluation: weight progression, digestive symptoms, reflux, complications, or other concerns related to the previous procedure.
Depending on the initial surgery and current situation, the surgeon can evaluate whether a revision, a conversion to another procedure, or a different approach can be considered.
The goal is not to assume that a new surgery is necessary, but first to understand what is happening and obtain an evaluation tailored to your current situation.

Dr. Israel Augusto González González is a surgeon who graduated from Universidad Anáhuac Mayab. He pursued his training in general surgery, then developed advanced expertise in various fields directly related to bariatric management.

His background notably includes advanced training in bariatric and metabolic surgery, as well as advanced endoscopic surgery.

This combination is particularly relevant in the context of bariatric surgery, since options are not necessarily limited to a single technique or a first procedure. Some patients begin their journey with primary surgery, while others consult several years after an initial surgery to have their situation re-evaluated.

Dr. González notably holds:

The analysis begins with a virtual meeting with an independent nurse who is a member of the collaborating medical team. Together, you will complete the necessary questionnaires, and the nurse will collect relevant information regarding your health, medical and surgical history, medication, weight loss history, lifestyle and dietary habits, as well as certain factors that may influence the risks associated with your project.

A requisition for blood tests and, when required, for additional complementary examinations is also provided to you so that the results can be included in the information considered in the analysis of your file.

The independent medical team thus has several elements to study your situation, notably your health and risk factor questionnaires, your blood test results, your BMI and weight history, your background, your medication, your eating habits, certain symptoms such as reflux, your previous surgeries, as well as your expectations and goals. If you have already had bariatric surgery, the information regarding that procedure and the evolution of your situation are also considered. Depending on your profile, additional information or examinations may be requested.

Based on the available information, the independent medical team analyzes your file and the risk factors relevant to your project. You then receive a report outlining the identified elements, medical recommendations, and, where applicable, conditions, optimizations, or additional examinations that may be recommended before moving forward.

This analysis also allows the medical team to consider the different bariatric approaches that can be considered according to your situation, rather than automatically starting from the procedure you initially had in mind.

This step takes place before the official booking of your trip. It therefore allows you to have more information in hand before booking your time off and starting to practically organize your stay.

Because a bariatric surgery project should not simply start with “choose your surgery and book your trip.”

A sleeve, a bypass, a bipartition, or a revision surgery are not necessarily suitable for the same individuals. Your health, weight loss journey, diet, background, and reality must first be considered by the relevant healthcare professionals.
